The IUPUI Department of Environmental Health and
Safety (EHS) has developed this training to ensure a safe work environment
and to protect the health and safety of IUPUI faculty, staff, and
researchers who are potentially exposed to airborne contaminants
or may occupy an oxygen deficient atmosphere. The Occupational Safety
and Health Administration (OSHA) promulgated a final rule 29 CFR
1910.134. The training applies to all IUPUI faculty and staff who
are exposed to airborne contaminants that cannot be controlled through
engineering or administrative controls and to employees who anticipate
wearing respiratory equipment during an emergency incident.
There are three portions to IUPUI’s Respiratory
Protection Training Program. In order to meet the requirements of
this program, you must:
- Complete the Respirator Medical Questionnaire Form - This form
can be found on ehs’s website at http://www.ehs.iupui.edu/ehs/indus_hyg_respiratorQuestionnaire.asp.
Print out and complete the questionnaire. Place the completed
questionnaire in a sealed envelope marked confidential and send
it to EHS at the Union Building, Room 043. EHS, under IUPUI Health
Services’ guidance, will review the questionnaire. If there
are questions regarding the questionnaire, you may be asked to
meet with IUPUI Health Services;
- Complete the Respiratory Protection On-line Training and pass
the quiz; and
- Complete Respirator Fit Testing - After medical approval is
granted and you have successfully completed the training and passed
the quiz, EHS will contact you to schedule the respirator fit
testing. The fit testing typically takes approximately one half
hour to 45 minutes.
If you have not already completed the medical questionnaire and
sent it to IUPUI EHS, please stop this training and complete the
questionnaire. Then you may continue this training module.
